Intimate knowledge of community resources for seniors
Advanced experience in patient advocacy, social work, or similar field

Requirements
All applicants must be prepared as follows:
Experience
Required - minimum of 2 years experience in your field.
References
Provide names, addresses and phone numbers of your previous clients, registries, agencies, or employers for at least the last five years.

Physical
Within six months signed by doctor stating "Free of communicable disease and of good health sufficient to provide services to persons with compromised health.".
Chest X-Ray or Mantoux method tuberculin skin-test
Within two years with negative results and signed by doctor or person administering test.
HIV Original Certificate
Four hours for RN and LPN
Two hours for CNA and HHA
HIV Update
Two hours for RN and LPN
One hour for CNA and HHA
Domestic Violence Certification
Required Background Screening
FDLE (Florida Department of Law Enforcement) background screening - REQUIRED
National LEVEL 2 Background Screening (FBI) - REQUIRED
