Requirements checklist

– Active Florida Driver’s License (or official State of Florida ID)

– Active Auto Insurance Card & Current Vehicle Registration

– Professional Liability Insurance

– Social Security Card

– Proof of Residency or Citizenship
-Legal Resident: Permanent Resident Card or Work Authorization-Citizen: Voters Registration, Passport or Birth Certificate

– Professional License for CNA, LPN, RN and/or HHA Diploma (40 hours minimum)

– HHA training must be received from a public vocational technical school or a licensed non-public career education school

– 30 Hours of infusion therapy for LPN

– Evidence of a one-time HIV/AIDS training – 1 Hour

– Assisting with Self-Medication Certificate – 2 hour minimum

– Active CPR Card
Certification must be received from an instructor that is approved to provide training by the American Heart Association or the American Red Cross

– Physical Exam
Must have been completed within the LAST 6 MONTHS and states that registrant is “free of communicable disease”. Given by a M.D., D.O., ARNP, PA, or RN super vised by a physician or acting under a protocol signed by a physician

– Level II Background Screening with eligible results – On file with AHCA

– Alzheimer’s Training Certificate – 1 Hour
